Joseph F. Waterer – Voluntary Leave Transfer Program

Joe a Flight Data Communications Special at Anchorage Center (ZAN) is having major surgery, and has been approved for the Voluntary Leave Transfer Program (VLTP). We strongly urge our members to donate leave to Joe, as always, WE ARE FAMILY! Below is the e-mail message from Joe, followed by the VLTP information.

I am soon going to have my third major back surgery with a spinal fusion to put me off work of approximately 6 months to a year any help from my union members in flight data would greatly be appreciated. I additionally will need another back surgery in the future after I retire. I have to lose some weight right now due to the risk of the surgery and I am doing this, also need to get my severe back pain and leg pain under control so there doing pain management right now so I can Hopefully return to work for a short period until the surgery, they have me on strong narcotics and doing steroid injections into my spine right now. It’s going to be a difficult time any help I appreciate.

Employees interested in donating leave may submit their donations through the Online VLTP Intranet site located @ https://webapps.awp.faa.gov/ovltp/new_enter_donorapp.cfm?here=nat&RecordID=3673. In order to donate, you must click on the "Online Leave Donor Application". NOTE: Donors must use a FAA Computer with Intranet access to donate leave through this process! The donation process is not available through the Internet.

Manual submissions must be submitted on FAA Form 3600-25, "Federal Aviation Administration Voluntary Leave Transfer Program Donor's Annual Leave and/or Sick Leave Application (Within FAA)". This form may be downloaded at the above website address and found under section entitled, "Forms".

This notice should be posted in a common area for all employees to review at facilities where employees do not have access to e-mail.

Forms should be forwarded to Patricia Brown, Leave Donor Coordinator, AAL-17, 222 W. 7th Ave., #14, (AAL-17) Anchorage, AK 99513, or faxed to me at (907) 271-4809.
